A regular wellness exam is one of the best ways to keep your pet happy, healthy, and thriving. This checklist will help you prepare for your appointment and ensure your veterinarian has all the information they need.

Eyes & Ears

Infections, inflammation, or discharge

Nose

Congestion, drainage or ulcerations

Mouth

Dental disease, fractured teeth, periodontal disease, oral infection or tumors

Lungs & Heart

Respiratory congestion or abnormalities, murmurs or abnormal sounds & rhythms

Internal Organs

Palpated for any signs of abnormalities or problems

Leg Joints

Range of movement, swelling, arthritis, instability or pain

Genital & Perianal Region

Signs of abnormalities, growths, swelling, discharge

Kidneys

Pain, sensitivity and size

Skin

Any sign of allergies, skin disease, lesions, abnormal bumps or growths

Fur

Ectoparasites such as fleas, ticks & mites as well as the condition of the coat

Please bring the following if available:

  • Previous medical records or vaccine history
  • List of current medications, supplements, or vitamins
  • Your pet’s current diet (brand, feeding schedule, and treats)
  • A fresh stool sample (within 12 hours) for parasite testing
  • A urine sample (if possible — use a clean container)
  • Any recent behaviour changes (e.g., appetite, energy, bathroom habits)
  • Questions or concerns you’d like to discuss with the vet
  • For new patients: proof of ownership (adoption records, microchip ID, etc.)


Your veterinarian will perform a complete head-to-tail assessment:

  • Weight & body condition scoring
  • Temperature, heart rate, and respiratory rate
  • Eyes, ears, teeth, and gums evaluation
  • Skin and coat condition
  • Lymph nodes and abdomen palpation
  • Muscle tone and mobility
  • Heart and lung sound check with stethoscope
  • Parasite check (fleas, ticks, worms)
  • Behavioural assessment and lifestyle review
  • Discussion about diet, exercise, and preventive care

Depending on your pet’s species, age, and risk factors, your vet may recommend:

  • Vaccinations (core and lifestyle-based)
  • Heartworm and parasite prevention
  • Fecal testing for intestinal parasites
  • Bloodwork (CBC, chemistry, thyroid, or senior profile)
  • Dental checkup and possible cleaning
  • Microchipping for identification
  • Spay/neuter or reproductive planning
  • Nutritional counselling and weight management plan

Older pets benefit from semi-annual wellness visits and additional testing:

  • Full blood and urine panels for early disease detection
  • Mobility and arthritis assessments
  • Thyroid and organ function testing
  • Eye and dental checks for age-related changes
  • Pain management or supplement recommendations
  • Review your pet’s treatment or vaccination plan
  • Schedule next visit or follow-up testing
  • Set reminders for parasite prevention and dental care
